An Adobe PDF of Dr. Okuda’s Elective Submission Form is available under the “Policies” tab above, or by clicking this link. Please fill it out and drop it off in his Sinai mailbox at least two weeks prior to the start of your planned elective.

New Elmhurst Roles & Zones
